Create 8D-Report

A 8D report is a document that will exchange in the framework of quality management in a claim between supplier and customer. 8D stand for the eight process steps that are required during the processing of a complaint to overcome the problem.

<note tip>This function is available since Version 4.2.</note>

Create out of an Incident

1) You are in your Incident, please click on Create 8D-Report at the <font color="darkorange"><b>I want to</b></font>-panel.

2) In next step you have to choose the Owner of the 8D-Report.
Please select an appropriate person in your system and then click on OK.

3) It opens a new window and you are now in the general overview of the 8D-Report.

The first tab shows a overview about the content of the 8D-Report. Moreover the <font color="darkorange"><b>attachments</b></font> and the already set <font color="darkorange"><b>measures</b></font> will be displayed.

Overview
ID The ID is given automatically by system when you create the 8D-Report out of an Incident.
owner The chosen owner one step before will be shown.
(mandatory field)
title This is the title of the Incident.
(mandatory field)
description Place for additional information.
status In Beginning the status is open. The status change to complete when the 8D-Report is finished.
planned end date The date when you want the 8D-Report should be finished.
Attachments
Here you can add some attachments to the 8D-Report.
Measureoverview
Here are listed all created Measures. To edit one you have to change to the corresponding discipline.

The following window opens where you can search for a CI.

Through Assign CI you can assign a CI and remove it through remove CI.

The desciplines

The 8D process includes the following steps in 1stAnswer:

  • D1 Add new teammember: Make up a team to solve the problem.
  • D2 Overview: In the overview of the problem is defined as precisely as possible.
  • D3 interim actions: Emergency measures intend to harm reduction and to prevent the further spread of the problem until a permanent solution is found.
  • D4 identified root causes: Errors are detected and identified the root cause.
  • D5 verified permanent corrective actions: It identify measures that can eradicate the root causes.
  • D6 permanent corrective actions to implement: The shut down procedures are introduced and the effect of emergency to be eliminated.
  • D7 measures for preventing recurrence: It must be ensured that the same or similar mistakes in future be excluded.

Teammembers

At the following part you can add teammembers to the 8D-Report. For this you can use the search to search for a person.
Through Assign Person you can add a new teammember and remove it through remove Person.
All added teammembers are listed up down on the page.

Create measures

Using the example of D3. The process steps D3-D7 is the same schematic.

owner Here you have to choose an owner for the measure out of the list of teammembers of the 8D-Report.

title Here you have to enter a title.
(madatory field)
description Place for additional information.
status At the beginnen the status is open. During the editing the status can change to failed[since version 5.3], completed and verify.
end date Here you have to enter until which date the measure should be completed.

After saving you see an overview of each process.

Edit measure

  • Edit measure: Through this symbol you can change the data of the measure.
  • Create a RfC: Through this symbol you can create a RfC. A Request for Change, include change requests and tasks collections which serve to solve the problem.
  • Delete measure: Through this symbol you can delete the measure.

After saving the measures all will be listed up on the overview page on the bottom.

The history displays all changes which are made at the 8D-Report.

At the <font color="darkorange"><b>I want to</b></font>-panel you can save all information through <font color="darkorange"><b>Save 8D-Report</b></font>. To close a 8D-Report click on <font color="darkorange"><b>Complete 8D-Report</b></font> and on <font color="darkorange"><b>Reopen 8D-Report</b></font> when you want to reopen it after completing. In addition, you can send the 8d-Report in a email to a specific person through <font color="darkorange"><b>Send 8D-Report</b></font>.

In this view you can decide which measure is sending with. Through Add measure you can add the measure and remove it through remove measure.

Following click on send 8D-Report to change to the email sending. Here you can change the text and after all send the email to the needed persons.

Edit 8D-Report

First you have to find the 8D-Report you want to edit. For this you have to go to the pool.

How the search works you can read on page The Search.

The result is showed as followed:

Out of the Pool you can open

  1. the 8D-Report through click on the Id, title or status
  2. the linked Incident/ Problem through click on the Id of the Incident/Problem
  3. the user profile through clicking on the name of the user.

When you open a 8D-Report you have the same editing opinion as described above.

Create out of the 8D-Report Pool

Through the button Create 8D-Report out of the pool you can create a new 8D-Report.

In Compare to the creation out of an Incident the mask is completely empty and has to be filled out. After saving all option explained above will be available.
